# How to Set Up Intake Forms in Picktime

Picktime Intake Forms help you ask customers extra questions before they book. Each answer is saved with the booking it belongs to.

**Path:** **Online Booking > Settings > Intake Forms**

***
## Before you begin

Intake Forms are used for questions related to the booking. For example, you can ask about the reason for today’s visit, allergies, pet breed, special instructions, or appointment notes.

Use [Customer Details](https://support.picktime.com/en/article/how-to-add-or-delete-customer-additional-fields-in-picktime-93z8xi/) for customer profile information, such as name, phone number, address, or birthday. Use **Intake Forms** for questions specific to a booking.

**Note:** Intake Forms are available on the **Starter** and **Pro** plans. You can add up to **20 questions**.

## Where Intake Form answers appear

After a customer completes the form, you can view their answers in the booking details. If you use booking approvals, the answers appear while reviewing the request. Intake Form answers can also appear in booking reports, printed bookings, and custom email templates.

***
## Good to know

Once a question is created, the question wording, field type, and choices cannot be edited. To change them, delete the question and add a new one. This keeps past answers matched to the question the customer saw when booking.

You can turn **Required** on or off anytime. This only affects new bookings.

If you delete a question, it stops appearing on the booking page, but past bookings still keep the answers already submitted.
