How to Set Up Intake Forms in Picktime

Picktime Intake Forms help you ask customers extra questions before they book. Each answer is saved with the booking it belongs to.

Path: Online Booking > Settings > Intake Forms

Before you begin

Intake Forms are used for questions related to the booking. For example, you can ask about the reason for today’s visit, allergies, pet breed, special instructions, or appointment notes.

Use Customer Details for customer profile information, such as name, phone number, address, or birthday. Use Intake Forms for questions specific to a booking.

Note: Intake Forms are available on the Starter and Pro plans. You can add up to 20 questions.

How to add an Intake Form question

Step 1: Go to Intake Forms

From your Picktime dashboard, go to Online Booking.

Click Settings.

Scroll down to Intake Forms and click + ADD.

Step 2: Add the field label

Enter the question you want customers to answer.

The field label can be up to 100 characters.

Step 3: Select the field type

Choose how customers should answer the question.

Picktime supports five field types:

  • Short answer
  • Long answer
  • Choose one
  • Tick the boxes
  • Select from list

Step 4: Save the question

After creating the question, click Save.

You can drag the question to change its order, mark it as Required, and save the page to publish the changes.

Where Intake Form answers appear

After a customer completes the form, you can view their answers in the booking details. If you use booking approvals, the answers appear while reviewing the request. Intake Form answers can also appear in booking reports, printed bookings, and custom email templates.

Good to know

Once a question is created, the question wording, field type, and choices cannot be edited. To change them, delete the question and add a new one. This keeps past answers matched to the question the customer saw when booking.

You can turn Required on or off anytime. This only affects new bookings.

If you delete a question, it stops appearing on the booking page, but past bookings still keep the answers already submitted.
